Care Coordination

Care Coordination means that our staff works closely with you and your healthcare providers. Our Care Coordinator  will ensure that not only you are up-to-date with any information or questions you may have, but that your other providers are as well. This means that the patient's needs and preferences are known ahead of time and that this information is used to provide safe, appropriate, and effective care to the patient. The main goal of Care Coordination is to organize delivery of your  services in the right order, at the right time, and in the right setting.

We share ways to manage chronic conditions and better your overall health through diet and fitness, setting goals, monitoring progress and preventing complications. You can have regularly scheduled phone visits with our Care Coordinator, and receive educational materials and referrals to informative, physician-approved websites.

 

Our dedicated nurse Care Coordinator will explain what comes next and help;

  • Make follow-up appointments

  • Assist with transportation

  • Explain health information and answer questions

  • Collect up-to-date information from all of your providers
